A look into Knee Pain

As postural alignment specialists and graduates of Egoscue University, our focus is on the position of the client’s load bearing joints. Our shoulders, hips, knees, and ankles bear the weight of our body when standing and moving upright. When we look at our client’s joint position, many of our client’s are surprised by their knee position. 

The load bearing joints of the shoulders, hips, knees, and ankles should align.

Try this:
Stand in front of the mirror
Look at the middle of your thigh bone that is named the femur. Then look at the middle of your knee cap.
Draw an imaginary line from the center of the thigh bone to the middle of the knee cap.
Does the knee turn to the outside of the body or does the knee roll toward the middle of the body?

If the center of your thigh and center of your knee cap are not in line, the rest of your load bearing joints are picking up the work for the uneven load. This uneven load leads to knee pain, arthritis in the knee joint, bursitis, and torn ligaments!

When knees turn out or roll toward the middle of the body, there is significant wear and tear on the knee joint. Imagine going up and down the stairs with the knee cap pointing outward. The friction and demand on the uneven knee joint will lead to knee pain. Over time to avoid the knee pain we begin to sell our house to live on one level or we take the escalator to avoid the movement that leads to pain. This avoidance of triggers will decrease the feeling of the knee pain, but does not address the cause of the knee pain.

Oftentimes the exercise we do to be healthy and support our heart, aggravates our knees and we begin to wonder why. If your knees turn outwards and you cycle or run for 30 minutes, that is 30 minutes that your knee joint is impacted and inflamed. Frequent wear and tear from a misaligned knee exercising on the elliptical, bike, or treadmill will lead to arthritis of the knee and the need for a knee replacement. The exercise is not the enemy, but the position of the knee cap in relation to the thigh bone and pelvis. 

How do we avoid knee replacements, ligament tears, and arthritis? We must consider the muscles that support the thigh bone (femur) and knee cap (patella). Through a postural alignment assessment, we will identify what is off in your body and which muscles need to be activated to bring your body into alignment. By correcting and supporting the position of the lower legs, you will continue to be able to tackle stairs and maintain your heart healthy cardio!

Arthritis and Aging

Many of us assume that as we age, there will be a certain amount of pain to endure. We think arthritis is a sure thing.  Or, is it?  And if so, is age really the only culprit?

First of all, what is arthritis?     It means “Inflammation, etiology unknown”.  Where does the inflammation come from?  There is the implied theory that we’ve just worn out our joints because we’ve lived so long!  If that were true, then everyone over a certain age would have arthritis or worn joints.  We know that knee pain, shoulder pain, hip pain, back pain comes from wear and tear.

Well, most often patients present with one side or the other hurting –I hear it all the time “thank goodness my other knee feels great!” This phenomenon alone debunks the theory that time on earth is dictating joint wear and pain.  While joint compression from age can be a factor would we really only have knee pain, shoulder pain, hip pain, and back pain on one side if pain was caused just by longevity?  No, both sides would have arthritis pain.

As Posture Alignment Specialists certified by Egoscue University, we evaluate bodies in pain all day every day– young and old, all sizes, shapes, injuries, surgeries, abilities and disabilities and condition levels. Most people come to us because they want pain relief – with knee pain, shoulder pain, hip pain, back pain, and ankle pain.  The simple and amazing thing is that they all have the same problem – their bodies are not properly aligned.  What your body needs is not youth but alignment… the way it was intended.  The earlier one understands the importance of alignment, the less arthritis has a chance!

Notice the alignment showing the ankles, knees, hips, and shoulders vertically and horizontally aligned, the pelvis is neutral, the spine has an s-curve, and the knees and feet point straight ahead. When this isn’t what is happening in our bodies – for whatever reason (accident, surgery, work posture) – some muscles begin doing too much and some not enough.  That is when bones are pulled out of alignment because of the compensation and then joints begin to wear unevenly.  This is true no matter what your age, however, the more years it goes uncorrected the more work one must do to unravel the compensations.

We need alignment so that our joints are properly “loaded” while we participate in as much weight bearing movement as we can handle, keep our range of motion in all joints full and active!  It’s not age, its alignment.  No matter what your age, we can help you retrain your body to be straighter and more functional without all the pain!

SPRING IS HERE!

Spring in Charlottesville is here and it’s gorgeous out there!  Our clients are enjoying these beautiful days outside in their gardens, resuming their outdoor walking and running regime, or spending their weekends on long bike rides.  Inevitably, they ask us, “I don’t want to tweak my back pain, shoulder pain, neck pain with my activities so are there e-cises that I should do before I….?

The answer is always: YES!

Your Egoscue menu is designed to make your body more functional, and there are specific exercises that should be done before and after you complete an activity.  These exercises will work on correcting and aligning your joints while preventing pain symptoms often associated with that activity.  This will keep the back pain, knee pain, and shoulder pain at bay so that you can do the sports you love! Aging and Egoscue

What’s trending at Pain Free Performance? Aging….

When I think of the most common conversation that pops up at our clinic these days…it has to be aging. At Pain Free Performance we are made up of a lot of baby boomers, people who are interested in staying active, working hard, retiring well, while enjoying life to the fullest. What I’m hearing (and feeling) is that we don’t like the wrinkles, the diminishing muscle tone, but especially the insinuation that we’re losing physical momentum and there’s nothing we can do about it.  Society has always been fascinated with youth…but weren’t we young not long ago?

Then, something stops us in our tracks. We’re still skiing, golfing, swimming, running but afterwards we can barely move. We twist or land hard doing things we’ve always done and ouch – back pain, heel pain, foot pain, neck pain – where did that come from?!  I can’t tell you how often I hear “I suddenly feel so old”, “I can’t believe I can’t ____ anymore” (just fill in the blank), or “My doctor says I have arthritis…”.

I encourage you NOT to buy into the theory that because you’re a little older, that activity is a thing of the past. There’s a great quote John Elder, of Egoscue University, used in a recent article “If you didn’t know how old you are, you’d have no idea how old you are…”! Forget the number! The best thing we can do for ourselves is keep moving.  Stay active.  Listen to our bodies.  They are amazing – even as we age! The human body has an innate and amazing ability to heal.

Our bodies, young or old, are designed to run, jump, play, swim, move and rest without pain. The pain is an alarm sounding that something in the musculo-skeletal system is out of whack.  Your head should sit squarely on your neck – your ears over shoulders which should line up over your hips, which line up over your knees and on down to your ankles.  If you look at yourself from the side in the mirror and this isn’t happening, you’ve got some muscles that are not doing their job properly.  This creates uneven wear and tear on the joints and is how all of the tendonitis, stenosis, bulging disks and tennis elbow stuff come into play. It could be a result of an injury (ie; putting more weight on one ankle after breaking the other-for years and years) or habitual poor body mechanics or posture (ie; hunched over your computer, phone, steering wheel or easel).  Muscles pull bones – either into or out of alignment…are you getting the picture?

Most often, the missing link to becoming pain free is getting your body aligned so that you can continue to do all of the things you love to do.  Everything else – the meds, the massage, the adjustment – are great, but temporary relief.  Egoscue – it’s real lasting relief. If you have back pain, plantar fasciitis, rotator cuff syndrome, knee pain, arthritis, or wrist pain – don’t blame age, blame alignment!!

We know how to teach you to retrain your muscles so that they are on track – waking up those muscles that aren’t doing enough and quieting those muscles that have been overworking – perfectly pulling your skeletal system back into the design intended and relieving the pain. GARDENING SEASON AGAIN!

Ah yes! It’s time for gardening and yard work.  Finally!  But after just a couple of hours of gardening, my back hurts.  We hear this at Pain Free in the spring, especially as people start getting more active.  They get back pain from what seems to be the simplest most innocuous movements.

We forget that the body is a stimulus-response system (everything we do and eve think has a muscular response in the body). If you’ve been sitting around all winter that inactivity created some less mobile areas in the body.  You may have been active all winter, going to the gym, lifting weights, hours of aerobics, skiing – active!  But moving into the garden we still hear “my back hurts!”.  Those winter activities are great and, even better, fun! However, they don’t get our bodies more functional. What I mean by that is joints lined up vertically and horizontally.  Moving that bag of topsoil requires different movement strength than your weight machine, stair machine, or elliptical. You need to be functional before you are active to stay pain free.

Take weeding for example, Most of us will sit on the ground and start pulling weeds. That sitting position already puts your back in a rounded, flexed position and once you start pulling weeds you force the back muscles to work in that disadvantaged position with no help from the hips and legs (because you’re sitting on them;). Ouch!

WHAT TO DO?!!

BEFORE: Think of your gardening and yardwork as a strength work-out and prepare for it like that.  Do a little warm-up before you go out to get your body functional, lined up vertically and equal right to left.  Try these e-cises:

AIRBENCH

CATS AND DOGS

ARM CIRCLES

ELBOW CURLS

DURING: It’s good to change positions.  Instead of sitting the whole time, kneel or work from your hands and knees, get up and walk around.  Try standing and bend from your hips for a bit.  Change positions every 10-20 minutes.  Your body likes a variety of movement and positions!

AFTER: When you’re done and feeling all proud of yourself, instead of plopping down on the couch, take that much deserved break in static back – everyone’s favorite exercise and so restorative.  If your back pain continues give us a call.  At Pain Free Performance, we are Posture Alignment Specialists certified by Egoscue University — we can help you eliminate that back pain and enjoy your garden!

Missy’s Story

CINDY MEYERS WRITES: A few weeks ago my client Missy stopped in with exciting news!  She had not been into Pain Free since February and she wanted to let us know that she was able to run for the first time in five and a half years!  Considering her physical condition at our first session, this was quite amazing news.  Missy had knee re-construction surgery in her twenties that left her with a very unstable knee.  She had back pain that radiated down her left leg and also suffered from right elbow tendonitis.  Analyzing Missy’s posture revealed that her left leg was bowed, her right leg turned in, her head forward with rounded shoulders and her pelvis uneven.  In her sessions, we worked to correct the underlying muscle imbalances that created the misalignment of her bones and her poor posture!  Here is what Missy has to say about getting her body back into balance:

Missy’s Story

Four years ago, at the age of 45 I was not able to tour Montpelier without my back aching during the one hour tour.  I could not stand still without bending and sitting during the lectures.  Two and a half years ago I could not walk my dog for thirty minutes on a trail in the woods without causing a backache that would last for days.  I had knee reconstruction twenty seven years ago and reinjured the knee several times over the years.  This caused knee instability, a bowed left leg, and backaches.  I was advised by an orthopedist to walk only for necessity, no hiking, touring, dog walks. He stated the arthritis in my knee was too far along.   I was also suffering from reoccurring carpal tunnel symptoms three years after having the “release” surgery, and tendonitis in my elbow from knitting.  I was only 45, in very good health otherwise, slim, active, medicine free. 

In preparation for a family trip to Bali, I started physical training with Pain Free.  I was concerned about the twenty four hour plane ride, because four hour car rides to Raleigh, NC caused me back pain.  I also wanted to participate in a seven hour volcano hike while in Bali.  I worked for several sessions with Cindy before my trip to Bali, and completed my menus daily at home.  The trip was a success. I did not need any pain medication to get through the two plane trips or volcano hike!!!  The carpal tunnel was also gone as well as the tendonitis. 

Eighteen months went by without my continuing the exercises.  I woke up one day and my reconstructed knee was completely out of alignment.  I could not walk.  I had to hop.  After a few exercises with Mackie, my knee was realigned and I could walk out of Pain Free.  Another family trip was on the horizon, this time seven days of waking around Rome.  Again I was unable to even walk the dog.  I started with Mackie this time and spent seven sessions working on strengthening my posture so that my knee would not fall out of alignment while asleep in Rome.    Rome was a success-no backaches, no knee pain, no pain meds, and no swollen knee. This time I have continued daily with my various exercise menus and have had no reoccurrence of the problems. 

I ran for the first time in five and a half years this fall.  Before that, I was not able to take a running step without my knee collapsing.  However, without thinking one day, I ran to catch up with a group of friends.  My family and I are amazed!!! It is all the good professional work of the folks at Pain Free.  My family and I are 100% convinced that without Pain Free, these trips would not have been possible for me.  My knee is so misshapen now that, while only 50, I am a candidate for knee replacement surgery.  But, with the daily practice of my Egoscue exercises and professional help of the folks at Pain Free, I have been able to resume an active life with my family without knee surgery.

ARTHRITIS

Perhaps you’ve visited your physician lately because of joint pain, and she tells you that you have arthritis.  Maybe she mentioned “degenerative joint disease” – which sounds even worse!  Well, these two things are basically the same thing – inflammation of the joint.  Many people think that the arthritis diagnosis is inevitable especially as we get older, not realizing that there is something to do to correct it, not just mask the symptoms with pain relievers. 

There are several different kinds of arthritis but we are talking about Osteoarthritis (OA).  This is the most common form of arthritis.  It is often called “wear and tear” arthritis and happens when cartilage in the joint breaks down creating the painful symptoms and joint swelling.

Sometimes OA presents on just one side of the body and not the other.  You might wonder why since both sides of the body are the same age! It all has to do with joint position.  When we are out of alignment (perhaps due to an injury) then we “load” one side of the body differently than the other, favoring the injured side.  When this happens, then that side will “wear” differently and faster which begins the arthritic process.  Sometimes people with osteoarthritis experience pain in all of their joints.  When joints are not lined up, the surfaces of the joints wear unevenly creating friction.  This friction causes inflammation, pain, and stiffness ….arthritis

One of my clients has arthritis in her hands…stiff sore inflamed joints-ouch!  But it’s not really about her hands at all.  Her shoulders are rounded forward, causing her elbow and wrist to be out of alignment.  All the joints are not moving well together and there is friction in every text and email she sends.  So, we’ve got to get her shoulders back into proper alignment, which will get her elbows in place and on down to her wrists and fingers. 

At Pain Free, we teach people how to get their bodies back to the original design – joints back in an equal bilateral position and the rest of the body aligned in strait lines, at 90 degree angles to the line of gravity.   So, don’t despair if you are experiencing arthritis pain – the healing power of your body is amazing!  You just need to know what to do and we’re here to help.
